What Should Your Expectations Be From Your Appointments With MediWellness?

First Visit

The Initial Appointment lasts approximately two hours. It consists of an in-depth interview focusing on your current symptoms, your medical history, lifestyle issues and dietary habits and a complete physical examination. Typically, at the end of the first visit, blood work and other pertinent specialty lab testing will be ordered.

Follow –Up Visit

The first Follow-Up Appointment usually takes place 2 weeks after your Initial Appointment and also lasts approximately on hour. At this time we will review your test results in detail and start you on a comprehensive treatment program. The first follow up visit will also include ample time for you to ask questions and to gain a solid understanding of the treatment plan that has been designed for you.

Subsequent Follow Up Visits will be scheduled as necessary, based upon your individual healthcare needs.

Telephone Consultations can also be arranged.

Fees and Insurance Coverage

Appointments

Initial Visit: $360.00
Follow Up Visit: $195.00

Due to the complex nature of integrative medicine, which demands more lengthy and in-depth office visits than the standard 15 minute visit offered by most insurance plans, MediWellness is unable to offer direct insurance or Medicare billing; however, MediWellness will provide you with an insurance super bill that you may send to your PPO insurance provider for reimbursement.

Charges for all office visits are due and payable at the time of service. Checks, cash, VISA and MasterCard, Discover and American Express are accepted.

Additional Testing and Paperwork

The cost of laboratory work and any other additional, pertinent specialty testing is not included in the cost of the office visit. Most laboratories and radiology centers that we use do offer direct billing to your insurance company.

The completion of additional paperwork by MediWellness physicians and other health care providers (such as letters to your medical insurance company) is billed at the rate of $320 per hour.

Scope of Practice

Please be advised that MediWellness services are intended to complement those provided by your primary health care provider. As consultants in Integrative Medicine, MediWellness health care provider are in the office appointment only and are not available to handle urgent or emergency medical situations. In the event of sudden illness or emergency, patients are expected t contact their primary physician an/or go to the nearest Emergency Room. We appreciate your understanding.

To Make an Appointment

1. Download New Patient Forms

In preparation for your appointment, please download the new Patient Forms from this website (see below), complete the forms, and bring them with you to your initial visit.

2. Bring Recent Test Results

If you have had laboratory or other medical testing done within the past 12 months, you are encouraged to bring the results/reports with you to your initial appointment. This may partially eliminate the need for us to order new tests for you and thus may reduce your total cost for services.

3. Bring Your Medications and Supplements

If you are already on a medication and/or supplement regimen, you are also encouraged to bring these medications/supplements with you to your first visit so they can be reviewed by Dr. Levy.
